Your First Visit With Dr. Blalock
When preparing to visit Dr. Blalock for your initial visit, we will reserve two hours of our schedule to complete a thorough examination and allow time to discuss your treatment options.
Your first visit includes
- A review of your complete dental and medical history forms.
- Complete X-rays of your teeth and mouth.
- An oral exam.
- A dental consultation and answers to any questions you may have.
Please bring
- Your completed Health History Forms.
- Your insurance card.
If we have forms and a copy of your insurance card before your visit, we can do a complimentary review. When you arrive, we can give you the needed information and spend more quality time with you.

The American Dental Association recommends that you and your child visit the dentist every six months for regular checkups and routine teeth cleanings. We would love to see you here every six months to make sure your smile is staying healthy.
